Chronic Condition Management Plan

A plan organised by your GP for allied health support in managing your condition or for prevention of further conditions. The plan currently offers 5 subsidised allied health appointments each year. So, you could see a podiatrist once, an exercise physiologist twice and a dietitian twice in one year. Or you could see a dietitian five times in one year. It all depends on what you and your GP think would be most beneficial to managing your health.

This plan is used for clients with chronic conditions e.g. diabetes, irritable bowel syndrome and other gut-related conditions, high blood pressure, high blood glucose, high cholesterol, liver disease, heart disease, coeliac disease, endometriosis, polycystic ovary syndrome, disrupted eating patterns and metabolic risk factors.

Eating Disorder Plan

An Eating Disorder Plan can be organised by your GP or a psychiatrist if you meet the medical criteria for an eating disorder diagnosis.

The plan currently offers 20 dietitian visits and 40 psychologist visits per year. The plan helps coordinate wrap around care for people experiencing specified eating disorders.
